Blowout Fracture and its Rhinological Approach

　I．緒言

　表題に掲げたblowout fractureは眼窩壁骨折のことであるが，本症発症の機転を極めて如実に表現しているために，あえて日本語訳名を使用せずに英語名のまま広く使用されている。

　このblowout fractureの名を初めて提唱したのはSmith B.（1957）¹であるが，比較的に鈍的な外力が眼部に加わり，眼球が内部に押し込まれることによって眼窩内圧が上昇し，眼窩下壁ないし内壁を押し破って眼窩内容が副鼻腔に吹き出し嵌頓するものとされている。本邦においては本症は眼科，耳鼻咽喉科，顔面外傷を取り扱っている形成外科などで治療され，その結果が報告されているが，本症の治療に関して，それぞれの立場からの主張があり，必ずしも統一された見解が確立されていないようにも思われる。最近の筆者らの経験例からの見解を述べてみたい。

　Six cases of orbital blowout fracture treated by transmaxillary approach are reported.

　A bone plate obtained from the canine fossa was used as a graft on the defect of the orbital floor with good results. A balloon catheter was useful to fix the bone graft at the position. In one case double vision at the upper gaze was remained after the surgery. Discussing the reason of this insufficient result, it is concluded that immediate surgical reduction should be carried out when the incarcerated tissue has the risk of necrosis. The radiological findings in such condition was also demonstrated.
