Prognosis of 46 Surgically Treated Cases of Blowout Fracture of the Orbit

緒言

　Blowout fractureは眼窩縁に強い衝撃を受けることなく，直接眼窩内容に強い外力が働いた場合，眼窩壁の中で最もうすく，弱い眼窩下壁が，急激に上昇した眼窩内圧のため，下方の上顎洞内にぬけるように骨折をおこすものをよんでいる。このような発生機転とともに，受傷直後より著明な眼球の上下方向に対する運動障害をおこすことの多いことで，眼窩骨折の中でも特異なものといえる。

　本症に対する治療は眼筋の機能障害を治す意味において，できるだけ早期に嵌屯した外眼筋を含めた眼窩内容の整復と再脱出の防止につきる。この目的のため，従来から眼窩側および副鼻腔側から嵌屯組織の整復を行ない，また骨欠損部の修復に自家骨片，Silicon板などを用いる手術法^1）4）が多く発表されている。

The post-surgical course is evaluated in a consecutive series of 46 cases blowout fracture of the orbit during the past 6 years. Surgical correction of the fractured orbital floor with implanatation of silicone plate could be per-formed within 15 days after the trauma in 17 cases (37%), within one month in 7, within 3 months in 11 and later than 3 months in 11 instances.

Remarkable improvement in eye movement and disappearance of diplopia generally resulted when surgical correction was performed within 15 days after injury, 47% of the cases show-ing complete and the rest incomplete cure.
