Blowout fractureの臨床経過と治療

The clinical course and treatment of blowout fracture of the orbit

　1978年6月より1979年6月まで，昭和大学眼科で手術を行なった，眼球運動障害を有した，Blowout fracture　60症例につき，その臨床経過を検討した。臨床症状の特徴ある所見としては，受傷後，日を送るに従って眼球陥凹が著明になることと，受傷後4週間以上を経験した症例16例にも，明らかな眼球運動障害を認めた点である。また手術後の症状の変化としては，眼球運動障害は88％に消褪または軽快をみているのに対し，眼球陥凹の改善度は低かった（48％）。

　手術によって眼球運動障害の増強した症例のみられないことを合せて，著者らは，眼球運動障害を有するBlowout fractureは，早期に手術療法を行なうべきであると考えている。

A retrospective analysis was made on 60 cases with blowout fracture of the orbit who underwent surgery during the 13-month period ending June 1979. All the cases had impaired ocular motility.

There was a definite tendency for the enophthal-mos to become more manifest with the lapse of time after injury. Contrary to the accepted view, impair-ment in ocular motility was still present in all the 16 cases who visited us after 4 weeks or more after injury. Surgery brought about normalization or improvement in ocular motility in 88% of the cases. Surgery was generally futile in improving enoph-thalmos.
