Blowout fractureの経上顎洞および経眼窩整復について

Transmaxillary and transorbital approach to the blowout fracture of the orbital floor

　1972〜1981年の10年間に，blowout fractureの症例に対して，経上顎洞法（11例）と，経眼窩法（21例）で手術を行い，その成績を比較検討した。両者とも約70％で複視の消失が見られれたが，眼球陥凹に対しては，ほとんど効果が見られなかった。経上顎洞法は術野が広く，眼窩底骨折部への嵌頓組織の整復が行いやすく，もっと試みてよい方法と考える。

We evaluated the postsurgical outcome of blow-out fracture of the orbital floor by transmaxillary approach (11 cases) and transorbital approach (21 cases) during the 10-year-period (1972-1981). After either procedure, diplopia disappeared in 70% each. Enophtahlmos generally persisted after either procedure.

As a noteworthy merit of transmaxillary ap-proach, the site of fracture of the orbital floor was better exposed facilitating surgical repair. While the postsurgical outcome is about the same for transmaxillary and transorbital approaches the former is recommended for the above reasons.
