I．はじめに

　1957年Smithら^1）が，眼窩下壁骨折の特異な発生機序を報告して以来，blowout fractureに対する外科的整復が積極的に行われるようになり，特にシリコン板等，種々の眼窩壁補填材料を使用した整復術が急速に普及してきた。しかし近年，それに伴う種々の術後合併症の報告，さらにはPutterman^2）のごとく，保存的治療を強調する報告がなされ，blowout fractureに対する不用意な外科的整復を反省すべき時期にきていると思われる。

　我々は最近，blowout fractureの手術後6年を経て，下眼瞼腫脹と視力低下を来し，眼窩腫瘍が疑われた興味ある1例を経験した。本症例は，blowout fracture手術の晩発性術後合併症の1例と考えられ，その発生機序につき若干の考察を加えたので報告する。

　A pseudotumor (cholesterol granuloma) of the orbit in a patient complaining of swelling of lower lid and visual loss 6 years after surgical treatment oforbital blowout fracture was reported. The cystic mass was 25 × 20 × 15mm and contained a silicone plate inserted in the previous operation. Transmaxillary cystostomy was performed and resulted in the improvement of not only occular movement but also visual acuity.

　We considered this as one of late postoperative complcations following surgical repair of blowout fracuture using a silicone implant. In this case the silicone plate apparently injured the intraorbital blood vessels and formed an intraorbital hematoma postoperatively.
