　I．はじめに

　いわゆる良性発作性頭位眩暈positional vertigo of benign paroxysmal type（以下BPPVと略す）は，臨床上比較的多くみられる疾患であるが，その病因に関してはいまだ不明な点も少なくない。今回筆者らは昭和46年7月より昭和52年7月までの6年間に北里大学医学部耳鼻咽喉科メマイ外来を受診し，BPPVと診断された129例中，確実例65例について検討し，若干の知見を得たので他の文献的考察をあわせ，ここに報告する。

　In 65 cases of positional vertigo of benign paroxysmal type (BPPV), change of the direction of rotatory nystagmus appeared in five cases, and recurrence of the symptoms was found in other five cases in each clinical course.

　We supposed that the pathogenesis of BPPV may be due to insufficiency of vestibulocochlear artery which provides a blood supply to the singular nerve.

　It is also supposed that the change of the direction of nystagmus might be caused by recovery from the circulatory disturbance of vestibulocochlear artery.
