換気機能検査による喉頭領域狭窄障害の診断

DIAGNOSIS OF LARYNGEAL STENOSIS BY USING MEASURMENTS OF VENTILATORY FUNCTION

　I．はじめに

　上気道と下気道の接点に位置する喉頭領域は呼吸と発声の両機能を同時に有する器官であるために，従来の換気機能検査法では動的状態における喉頭領域の狭窄障害を検出することは容易ではなく，呼吸気道全体としてみた場合，もつとも検出しにくい部位であつた。しかるに近年，換気機能検査装置の開発と性能の向上はめざましく，ことにフロー・ボリューム曲線とクロージング・ボリューム検査の採用は主に中心部気道より上部の閉塞性障害の検出しかできなかつた従来の検査装置とあいまつて上・下気道の閉塞性障害の状態を的確に把握できることを示唆するものである。

　われわれの目的はこれらの装置の組合せによつて喉頭領域に狭窄をきたすいくつかの疾患について，特徴的なパターンを見出し，鑑別できるかどうかを検討したものである。

　The purpose of this investigation is to measure the airway ventilation in patients with laryngeal stenosis by using a new apparatus for ventilatory function measurements, and to note whether or not any characteristic changes could be found in various other pathologies causing laryngeal stenosis.

　The study was made on four separate groups; group of normal subjects for control; laryngealstenosis group; laryngeal polyposis group and laryngeal paralysis.

　The measurements consisted of respiratory resistance, spirometric values. flow-volume curve and closing volume.

　The laryngeal stenosis group showed through the time dependent parameters significantly different picture from that of the control subjects.

　No significant change was noted in cases with laryngeal polys.

　In cases with recurrent paralysis obstruction impairements of the small airways were noted.
