TURリゼクトスコープを用いての声門下狭窄の治療経験

A Case Report of Therapy of Subglottic Stenosis Using TUR Resectoscope

　はじめに

　声門下気管狭窄の原因として，気道火傷，腐食性ガス吸入，カニューレや挿管チューブの留置によるもの，ケロイド体質などが挙げられる^1）。これに対する経尿道的（TUR）リゼクトスコープを用いる治療法はすでに竹林ら^2），平良ら^3）によって報告されている。今回われわれは声門下約2〜3cmで狭窄をきたした原因不明の症例に対し，TURリゼクトスコープを用いて病変を除去し，その後長期経過観察を行い良好な成績を得たので報告する。

A 54-year-old female with subglottic stenosis underwent laryngomicrosurgery using a TUR resecto-scope. She has been complaining of dyspnea andstridor. She has no history of tracheostomy or tra-cheal intubation, and she has been treated as asthma. For treatment of subglottic stenosis, she underwent 2-stage operation consisted tracheos-tomy and laryngomicrusurgery. For laryngomi-crosurgery, a TUR resectoscope was used to remove the stenotic lesion. After the operation, T-tube stent was placed for prevention of stenosis. TUR resecto-scope was found to be useful for this kind of surgical intervention.
