細胞像よりみた慢性副鼻腔炎手術創の治癒機転について

PROGRESS OF HEALING IN THE NASAL SINUS CAVITIES FOLLOWING OPERATIVE TREATMENT OF CHRONIC SINUSITIS EVALUATED THROUGH THE CHANGES IN THE CELL DISCHARGE

　Ⅰ．はじめに

　慢性副鼻腔炎の根治手術が症例により多彩な経過をとるのは，慢性副鼻腔炎という複雑な病変がそれぞれ，その治癒機転に影響を与えているためである。すなわち手術方法や，その巧拙のみならず成因と関係したその病変の性質，洞発育という素因的なものなどいろいろな要素が介在していると思われる。

　筆者らは慢性副鼻腔炎の根治手術後の術創の治癒経過を，分泌物中の細胞像を経時的に観察して術創に生ずる肉芽の治癒機転と，その肉芽の性状あるいは術前の病変などと比較検討して興味ある知見を得たので報告する。なお成績は第65回日耳鼻総会シンポジウム「副鼻腔炎手術後の治癒機転をめぐりて―治癒機転と後治療―」で一部発表したものをまとめたものである。

　That radical operation for treatment of chronic sinusitis is an absolute guarantee for a complete cure of this malady has yet to be attained. This difficulty of attaining a complete cure in all cases may be reflected from the fact that the process of wound healing in chronic sinusitis greatly differ from that of the wound healing, elsewhere, in general.

　Comparisons were made in wound healing following radical operation on chronic sinusitis on one hand and on normal sinus on the other, by means of studying morphological changes in the cells found in the sinus discharges.

　Wound healing in normal sinuses was generally completed in about 5 weeks, whereas in sinuses affected with chronic sinusitis it was delayed for much longer period.
