喉頭癌の機能保存療法の検討

CONSERVATIVE THERAPY OF LARYNGEAL CANCERS

　I．はじめに

　喉頭部分切除法の安全性が，技術的にも治療成績的にも認められるようになつて^1），すでに十数年が経過した。われわれの教室でも本法を採用し，すでに60例に達したので，術前状態とその後の遠隔成績とを検討することにより，本法の適応方針の是非を検討した。また，放射線単独治療も適応を選べば，その成績の良いこともすでに報告されているので^2），われわれも厳選してこれを用いてきた。これら放射線単独根治症例の予後も検討することにより，その適応の是非も併せて検討した。

　Seventy laryngeal cancers, that appeared to be possible to preserve laryngeal function by partial laryngectomy or by irradiation therapy, were studied. Among these patients there were 43 individuals who had survived over the period of 5 years and the total survival rates for these patients were 93%. A few patients underwent partial or total laryngectomy because of recurrence. In early laryngeal cancers like T₁ or T₂ the patients should be initially treated by conservative therapy because in cases of recurrence a total laryngectomy may save the patient's life.
