耳鼻咽喉科領域の血管腫症例に対するCryosurgery使用経験

CRYOSURGERY AND ITS APPLICATION TO HEMANGIOMAS IN OTOLARYNGOLOGICAL FIELD

　I．はじめに

　凍結術はすでに19世紀の後半より，これを臨床上に用いる試みが行なわれていたが，Fay（1939）^1）が脳の悪性腫瘍破壊に凍結術を応用したのを始めとして，次第に臨床上に用いられる機運にあつたが，冷却剤に適当なものが選択されなかつたために，その臨床的応用は進歩が認められなかつたが，Cooper（1961）^2）が液体窒素を用いたLind cryosurgical unitを使用して，パーキンソン氏病の患者にcryo-thalamectomyを施行して，従来の手術手技に比較して卓越した手術成績をえてから，臨床各科においてその手術手技が取り入れられるようになつた。耳鼻咽喉科領域ではCahan（1965）^3）が始めてこれを扁桃に応用してより，多くの研究者によつて凍結術の基礎的事項ならびに臨床応用例が拡大されてきており，現在ではメニエール病の外科的治療法として^4），また鼻咽頭出血の止血法として^5），鼻咽喉頭の良性および悪性腫瘍の治療法として^6），また口唇，舌，口腔粘膜，口腔底の腫瘍の療法として^7），あるいはglomus tumorの摘出術^8），pain clinicへの応用例^9），また扁桃の手術例としてその応用範囲が拡大されてきているが，本邦での，耳鼻咽喉科領域疾患への応用は，わずかに井街・他^10），八木・他^11），法貴^12）をはじめとして，著者らの一部^13）によつて各種疾患に対する応用例の成績が発表されだしたに過ぎない。なかでも血管腫，乳頭腫，限局性の癌腫には，特にcryosensitivityが高い事が確認されてきているので，今回著者らは，耳鼻咽喉科領域における血管腫症例のみを選び凍結術を施行し，その経過を観察した所，従来の放射線療法，外科的手術方法に比して，すぐれた成績をえたのでここに報告し，ご批判をえたい。

　Surgical treatment of localized expansive growths such as hemangioma with its massive hemorrhage during the procedure has remained a significant problem to the otolaryngologist. Cryosurgery in these cases is suggested as a method by which the hemorrhage may be circumvented and allows a completely controlled excision.

　Cases treated under this method included hemangioma of the tongue, mucous membrane of the mouth, the palatine tonsils, the retropharyngeal wall of the hypopharynx and the external ear canal. The postoperative healing of the wounds was satisfactory with minimum scar formation.
