I．緒言
咽喉頭頸部の不定愁訴を訴えて耳鼻咽喉科を受診する患者は近年著しく増加しつつある1）。これらの患者を診察するにあたつてもつとも重要なことは，その中から器質的疾患，特に悪性腫瘍患者をいかにとりこぼしなく見つけだすかということである。日常臨床において咽喉頭異常感を訴える患者に対する精密なルーチン検査の確立が切望される所以である。
われわれは咽喉頭頸部に関する症状を訴えて来院した患者全員について喉頭高圧レントゲンを撮つて検討しているのであるが，ある種の疾患で喉頭高圧側面X線像において頸椎気道間距離の拡大することに注目し，計測方法ならびに正常値について先に報告した2）。その中で，第6頸椎体中央の高さにおける計測で平均値11.8±0.3mmを得て，その拡大判定の基準を13mmとすることを提唱した。その後日常臨床においてこの基準値に基づいて判定をくだしているのであるが，この基準値の妥当性についてはなお検討を要する。今回は22名の頭頸部腫瘍患者についてその病巣のひろがりと頸椎気道間距離計測値との関係について調査し，拡大判定基準値の妥当性について考察を加えた。
Authors noted that there ware certain enlargements in the cervical vertebro-tracheal distance occurring in some disease and proposed 13mm as criterion of the enlarged distance. Results of observations on 22 subjects are reported:
(1) Tne minimum and maximum of the measurements ranged 13 mm to 23mm.
(2) It is not so rare to find the thyroid tumor to be extending into the cervical vertebro-tracheal space.
(3) The distance of 13 mm is considered appropriate in adopting as the minimum of the enlarged distance.
(4) Even when no abnormality is found under indirect laryngoscopy, the patient who reveal increase in the cervical vertebro-tracheal distance under high voltage laryngeal radiography should be examined more minutely.
