Ramsay Hunt症候群における顔面神経麻痺に対する顔面神経減荷術の意義について

THE SIGNIFICANCE OF DECOMPRESSION OPERATION ON THE FACIAL NERVE IN RAMSAY HUNT SYNDROME

　Ⅰ．緒言

　耳帯状疱疹症，いわゆるRamsay Hunt症候群は日常臨床においてそれほど稀な疾患ではない。その多彩な神経症状はきわめて特徴的であるが，その症状発生のメカニズムは1907年Ramsay Hunt^1）によつて，はじめて病理学的に明らかにされた。本邦においては，1930年佐藤^2）によつてはじめて報告されて以来，今日まで数多くの症例報告がなされている。耳帯状疱疹症にみられるさまざまの症状の中でも，顔面神経麻痺は患者に与える心理的社会的影響を考えるにもつとも重大な問題である。われわれは九州大学耳鼻咽喉科臨床において最近経験した症例より，異なつた経過をとつた4症例を報告し，あわせて本邦で発表されている文献により顔面神経麻痺について統計観察を行なつた。近年，Ramsay Huntによつて提唱された「病変の中心は膝神経節にある」とする概念は組織学的に否定されてきており，従来その適応を疑問視されていた顔面神経減荷術を積極的に行なおうとする動きがある。われわれは，今回報告した4症例中の第1例に顔面神経減荷術を行ない興味ある所見を得，その経験に基づいて顔面神経減荷術の適応に対する考察を加えた。

　It has been generally considered that the decompression operation is of little use for facial palsy caused by herpes zoster oticus. In this paper four cases of this disease in which facial palsies passed through different courses are reported. In one of them the decompression operation was performed with good results where abnormality of the Fallopian canal was revealed. One hundred cases of herpes zoster oticus are reviewed.

　(1) Twenty five cases (30%) showed no recovery of facial palsy.

　(2) Anatomical anomalies such as abnormal course or stenosis of the Fallopian canal may interfere with recovery of the paralytic facial nerve.

　(3) Tactless waiting for recovery may cause missing of the chance for decompression operation.

　(4) Owing to the technical advancement in facial nerve surgery nowadays it has become possible that the total decompression operation may be performed without any undue surgical injury.

　(5) The concept that the geniculate ganglionitis as being the cause of herpes zoster oticus has been questioned by many authors.

　In consideration of the above mentioned facts the authors propose the decompression operation should be carried out actively in patients in whom facial palsy had continued for several weeks with-out any signs of abatement.
