耳鳴にたいする内耳麻酔の臨床応用

INNER EAR ANESTHESIA FOR TREATMENT OF TINNITUS

　I．はじめに

　耳鳴の成因，治療法に関してはまだ不明な部分が多く，とくに感音性耳鳴にたいしてわれわれは治療効果の少ないことに悩まされることが多い。旧来の薬物療法に代わつて最近，米山^1）の大後頭神経ブロックや安田^2）のキシロカイン静注法などが報告されてある程度の効果をあげている。後藤（敏），永浜^3）は感音性耳鳴の成因について蝸牛から皮質に至る聴覚路のいずれかの部位の刺激状態と考えているが，前庭にも同様の病態でめまい発作を頻発する内耳病変が存在する。このような前庭終末器の異常興奮^4）あるいは過敏状態にたいして，局麻剤の中耳腔注入による内耳麻酔が一時的な効果をもつので，内耳性めまいの病態推定の資料に用いられている^5）。内耳麻酔を行なう症例の中には，前庭症状の他に耳鳴，難聴などの蝸牛症状を伴うことが少なくないが，耳鳴が内耳麻酔で変化することがある。そこである種の耳鳴にたいしては発生部位の推定や，治療方針など，耳鳴にたいしても臨床応用の余地が考えられるので，内耳麻酔を行なつた症例中耳鳴を随伴していた症例を選び出して耳鳴の影響を検討した。

Effects of the anesthesia of the inner ear for treatment of tinnitus were studied in 22 patients by perfusion of 4% xylocain into the tympanic cavity through the tympanic membrane. The majority of the cases were accompanied by peripheral vestibular lesion, except in 4 who appeared to be free of such symptoms. The degree of tinnitus was evaluated by subjective symptoms as well as by tinnitus masking level measured by pure tone audiometry.

In 3 cases the tinnitus and vertigo were, both, totally reduced by perfusion for more than 24 hours; reduction of loudness was obtained in another 3 cases. No effect of the treatment was seen in 16 cases.

Clinical evaluation of the treatment showed that it was effective in 6 cases; 2 cases showed irritable stage of Ménière's disease and 4 cases were involved with paroxymal positional nystagmus with cochlear symptoms.
