興味ある耳小骨異常を認めた鼓膜正常の伝音難聴

Interesting Cases with Conductive Deafness due to Ossicular Abnormality

　I．はじめに

　鼓膜所見が正常な伝音性難聴症例としては，耳硬化症，頭部外傷による連鎖離断，耳小骨の先天奇形などがある。最近ティムパノメトリーが普及するにつれて中耳腔の病変の推定がより確実に行われるようになったので，このような症例の手術例数が増加してきた。最近われわれは，手術によって耳小骨連鎖の異常による伝音難聴であることが確認できたが，鼓膜その他の臨床所見からは，明らかな外傷，炎症，奇形などと確定するに至らない4症例を経験したので報告し，成因について若干の考察を加えた。

　Four interesting cases of conductive deafness with normal tympanic membrane were reported. Exploratory tympanotomy revealed various abnormalities in the incus or stapes, such as hypermobility of the incudostapedial joint, absence of the long process of the incus, and dissolution of the crus of the stapes. Satisfactory post-operative hearing improvement was obtained in 3 cases. Etiology and pathology of these cases were discussed.
