前庭水管・内リンパ嚢病変と内リンパ水腫—ヒト側頭骨11耳の観察より

PATHOLOGY OF THE VESTIBULAR AQUEDUCT AND ENDOLYMPHATIC SAC IN ENDOLYMPHATIC HYDROPS

　I．はじめに

　山川^1）やHallpike^2）以来の側頭骨病理研究のつみ重ねは，メニエール病と内リンパ水腫の関連の深さを形態学的に証明してきた。そのうちの多くは内リンパ水腫と共に内リンパ嚢に病変を見出しており，Kimura^3）の内リンパ管閉塞実験による内リンパ水腫発生の事実ともあわせて，内リンパ嚢の機能不全がメニエール病の一因としてクローズアップされている。

　われわれはコロラド大学側頭骨バンクプログラムで集められたヒト側頭骨約300例中組織学的に内リンパ水腫の存在を確認した側頭骨の内リンパ嚢とそれを収容する前庭水管病変を検索して内リンパ水腫の発現における意義について検討を加えた。

　Eleven temporal bones (10 cases) with histological endolymphatic hydrops were studied. Primary sites studied were the vestibular aqueduct and endolymphatic sac including measurement of the proxysmal rugose portion, and graphic reconstruction of the vestibular aqueduct. Four ears showed hypoplastic vestibular aqueduct and endolymphatic sac. Three ears showed endolymphatic sac pathology such as flattened epithelial cells, increased connective tissue and small lumen. A ear showed bony obliteration of the vestibular aqueduct at the isthmus portion presumably due to bacterial labyrinthitis. Significance of these pathological findings as the etiology of the endolymphatic hydrops were discussed.
