脳神経症状を呈したTornwaldt氏病の1症例

A CASE OF TORNWALDT'S DISEASE WITH CEREBRAL SYMPTOMS

　I．緒言

　Tornwaldtは1885年bursa pharyngea（咽頭嚢）の病変による臨床症状について記載し，以来bursa pharyngeaの炎症，嚢腫，膿瘍はTornwaldt氏病と呼ばれるようになつた。ところが現在までのTornwaldt氏病の症例報告を詳細に調べてみると，その中にはbursa pharyngeaのみでなく，咽頭扁桃中央窩（ressessus pharyngea medius）に形成された潴溜嚢腫，あるいは膿瘍なども包含されているので真のbursa pharyngeaの病変の頻度については判然としない状態である。しかしYankauer^1）は155例の鼻咽腔膿瘍を報告し，全例とも遺残アデノイド組織の化膿によるものであつたと述べているように，真のbursa pharyngea起源のものは稀なようである。本邦では貝田^2），妹尾^3），北村^4），渋井^5），道下^6）らに報告が見られ，そのうちbursaの症例は貝田^2），北村^4）らの2症例のみと考えられる。またその臨床症状を調べてみると，脳神経症状を記載した文献・成書はほとんど皆無といつてよい。著者らは右側IX，X，XI，XII，および左側X，XII，各脳神経の麻痺症状を呈したpharyngeal bursitis症例を経験したので報告する。

　Reports in the literature on cerebral symptoms on Tornwaldt's disease are rare indeed.

　The authors experienced a case of a man, aged 62, who complained of persistent, amlignant, occipital headache with a large amount of constant pussy postnasal discharge. He also suffered paralysis of the right vocal cord as well as that of the right sternocleidomastoid muscle and the root of the tongue.

　The patient was operated for the removal of the offending bursa by means of Bechmann's method with a remarkable improvement of symptoms.
