鼓室成形術における連鎖再形成について

　I．緒言

　慢性中耳炎の治療が炎症の消失のみでなく聴力の回復をも目的として鼓室成形術が一般に行われつつある。然しながら慢性中耳炎における病変は乳突においても，鼓室においても，鼓膜においても，耳骨連鎖においても，両内耳窓においても，耳管においても症例によつて夫々異つている。従つて鼓室成形術においては病変の観察とその除去においても症例によつて異つている。そのために広義の中耳の各部位を詳細にしなければならない。この点については既にActa otolaryngologica vol. 51，1959に記載しておいた。又聴力の回復を計るための伝音機構再建についても病変が症例によつて異る以上，その再建は症例によつて異らざるを得ないが，これを基本化したものにWullstein，Zollner，後藤，Ormerodの型分類がある。

　ここに記さんとする連鎖再形成の問題は，これらの型分類による手術法より一歩を進めて，連鎖離断のものにおいてⅢ型又はⅣ型の手術法を行う代りに，連鎖を形成して夫々Ⅰ，Ⅱ型又はⅢ型を行う問題について，如何にして連鎖を形成するか並びにその意義について考察を加えんとするのである。

In analysing 300 cases of tympanoplastyperformed recently for treatment of chronic otitis media the author noted, from the standpoint of hearing recovery, the importance of the presence of the ossicular chain of the middle ear as well as proper functioning of this structure. With this point in view the author attempted plastic repair of the chainstructure by various ways and means. It is found that, by animal experimentations the articular functions of the chain will be lost by such a repair and its function thereafter, will be reduced to mere changed columella action.
