聴力損失軽度な慢性中耳炎に対する鼓室成形術について

　慢性中耳炎は従来上鼓室型のものに対してのみ手術の適応とされていたが，鼓室成形術の発展と共にあらゆる慢性中耳炎が手術の対象となつて来た。而して慢性中耳炎はその経過の長引くと共に病変は一進一退して徐々に進行するものと考えられる。かかる考え方からすれば慢性中耳炎は早期に鼓室成形術によつて手術を行わなければならないという事になる。このために私^1）は既に慢性中耳炎の早期手術を唱えて，これについて記載報告した。

　然しながら慢性中耳炎の初期のものでは病変が軽度で，低音機構が比較的よく保たれていて，聴力低下の軽度のものがある。従つて慢性中耳炎の早期手術を唱えるからには，聴力損失軽度な慢性中耳炎が手術によつて術後聴力低下しないということが明かにされなければならない。前回の報告では6例について述べたが，今回は更にその後の症例の内，術前聴力低下の軽度なものについて術後聴力が如何になつているかを検査して見た。

Tymponoplasty performed on 11 cases whose hearing by speech test fell below 30 db due to chronic otitis media showed slight improve-ment of hearing. But, similar operations performed in 5 cases above 30 clb of hearing showed a marked improvement. However, as far as the improvement on the threshold of hearing is concerned in these two groups the former appeared to be the better. From the viewpoint of postoperative hearing improve-ment the question may be resolved into im-provement in the threshold of hearing per se rather than the degree in which the hearing recovery may be established. Tympanoplasty should he performed early in the course of chronic otitis media.
