根治手術後のものに対する鼓室成形術

　はじめに

　鼓室成形術は中耳に炎症のあるものでは，その炎症を消失せしめるのみでなく，伝音機構を改善して聴力をも回復せしめんとする手術的方法である。これに対して中耳根治手術は中耳の炎症を消失せしめて合併症の予防を計るにあり，保存的中耳根治手術は中耳の炎症を消失せしめて合併症の予防を計るのみでなく，耳骨，残存鼓膜を保存して聴力の保存を計るにある。

　従つて保存的中耳根治手術がその意図されるように目的が達成されるならば，古典的中耳根治手術はその必要が殆んどなくなって，相当に進展した真珠腫性中耳炎のみがその適応となって残るに過ぎない。しかるに保存的中耳根治手術が広く行われるようになつてからも，古典的中耳根治手術も広く行われて来た。この原因の第1は古典的中耳根治手術に習熟した専門医が多いということがあり，第2には保存的中耳根治手術による治療成績が必ずしもその意図する結果を得られないということによるものである。そのために鼓室成形術が広く行われるに至った最近までは，保存的中耳根治手術と共に古典的中耳根治手術も広く行われていたのである。しかも今日においても自己の習熟した手術方法によつて，中耳根治手術を行っている方が少くない。

The complaint of continued aural discharge or the lose of hearing was present in 122 cases in whom they were treated by either classical or modified mastoidectomy. Forty seven of these cases were subjected to re-operation. The advisability of tympanoplasty in such previously operated cases is discussed.

Literatures are reviewed with results that hearing after mastoidectomies classical or mo-dified is as a rule poor. The importance of tympanoplasty is all the more emphasized.

The author s' motive for adopting tympano-plasty was instigated through observations that a good postoperative hearing meant and emphasized the importance of tymapanic ca-vity and membrane.

Following radical mastoidectomy many cases are found with missing incus ; a fact' that should he well noted in attempting tympano-plasty.

Various problems of tympanoplasty applied to cases of re-operation are discussed with author's experience of 169 similar cases.
