頬部の腫脹を来した鼻咽頭類線維腫の2例

　I．緒言

　若年者にのみ発生する特殊の血管線維腫である所謂Nasenrachenfibroidは臨床例としては既に一般的な疾患となり，特に珍らしい疾患ではないが，腫瘍学の上からは興味ある腫瘍である。臨床的にも鼻咽頭に好発するためにその名を付されているが，一方には基底類線維腫Basalfibroidの名称もある如く，頭蓋底の骨より発生する点を強調されているがその発生の母組織となるものが何であるかについては必ずしも明らかではない。頭蓋底の蝶形骨又は側頭骨錐体等の骨縫合を埋める軟骨結合特にその残存幼若組織がこの腫瘍の発生母組織と推定されている。この腫瘍の発症が未だ明らかでないのも，この腫瘍の全体的な摘出が困難であり，又剖検例を欠くためである。この腫瘍はあくまでexpansivに発育するために骨の破壊を起さず，又青春期以後に発育を停止するために本症による剖見例に接しない。これらのために本症の発生病理は明らかでない。最近著者等は特異な所見を呈する本症例の2例を経験した。これらの症例は何れも頬に腫脹を来し，その1例は鼻咽頭には全く関係が見られなかつた。従つて鼻咽頭類線維腫の名称が当らない例であつた。これ等の例は何れもその発生部位を明確に知ることが出来たのと，又組織学的にもその発症を考える上に参考になる点が多いと思われる所見が得られたから報告する。この種の病像を呈する症例の報告は極めて稀なものであると思われる。本邦の文献で探したところでは佐藤氏の19歳の男子の報告（1956）が本症例のうちの第2例に似ているが，第1例に見られる如き翼口蓋窩に増殖して？の腫脹を来し，鼻咽頭に全く関係のないものは見当らない。

Murashima and Kobayashi report 2 cases of naso-pharyngeal fibroid that caused swel-ling of the cheek.

In the first case the tumor was found to be originating from the pterygoid process and no sign of its presence was seen in the nasopharynx. In the second case the tumor was found, also, to be originating from the pterygoid process but spreading over the pterygo-palatine fossa in the naso. pharynx.

Tissue examination of the specimen remo-ved from the first case showed some nerve fiber bundles among the cells that composed the tumor but there was no evidence that they were under pressure from the surroun-ding tissues. From this evidence the authors thought that there might be a connection between the perineurium and the origin of the tumor.

Clinical and microscopic aspects of the tu-mor are discussed.
