慢性中耳炎における鼓室内病変と聴力との関連について

THE RELATION OF HEARING ACUITY AND DISEASE CHANGES WITHIN THE TYMPANIC CAVITY IN CHRONIC OTITIS MEDIA

　Ⅰ．緒言

　中耳伝音機構のなりたちと，その病変時における聴力の障害のメカニズムの解明は，鼓室形成術の聴力改善手術としての手術法の基盤を形づくるものである。しかし，実際に個々の慢性中耳炎例を対象として，これを手術的に治療しようとする実地臨床医家にとつて，これら基礎的，実験的，または理論的な考え方はどのような意味を持つているのであろうか。どのように利用し得る，また利用すべきものなのであろうか。たとえばその一つの例として，術前何らかの聴力現象によつて中耳内病変の種類や程度を推察することができないだろうかという問題がある。

　そこできわめて実際的なこの問題に対して何らかの示唆を得ることを目的として，多数の慢性中耳炎症例について，理論的な考えと対比しつつ，鼓室内病変と聴力障害の特長との関連性を検討して見た。

　Observations were made on the operative results of 248 ears that had been affected with chronic otitis media and treated surgically for the period of the last 7 years. Middle ear ossicles in these ears were found to be presenting various conditions which were roughly divisible into 5 different types; normal, granulation formation, loss of movement, interrupted continuity and stapedial absence.

　It was found that a certain type of hearing loss revealed preoperatively often conformed to a particular type of ossicular disturbance that it was almost possible to predict the condition of the ossicles in which they may be found, by carrying out a careful hearing test before the operation, at the time of the operation.
