　吾々の行つている低血圧下手術

　新しい抗生物質の誕生は今や人類のあらゆる化膿性疾患を征服するかに見えたが，尚慢性副鼻腔炎は薬物による非観血的療法のみを以てしては万全を期し難く，手術的療法によらざるを得ない疾患としてのこされている。しかも，その手術成績は必ずしも良好とはいえず我々専門医の悩みは現在もなお存し，将来又続くのではなかろうか。

　慢性副鼻腔炎治療の目標は，篩骨洞の病的粘膜を如何にして徹底的に除去するかにあることはいうまでもない。長崎大学後藤敏郎教授は，この目標の為には，篩骨洞に上顎洞及び鼻腔より進攻すると共に必要なる場合は経前頭洞的にも進むべきであり，この綜合した手術によって全副鼻腔が線維性の再生上皮で覆われた相連絡する空腔となるのが慢性副鼻腔炎手術の理想であるとしておられる。私共もこの理想に共鳴し，最近に於ては上顎洞経由及び鼻内より篩骨洞，蝶形洞の病的粘膜の除去を行い，同時に，眉毛内切開によつて前頭洞より前篩骨部に侵襲する，即ちPansinectomyを施行し見る可き成果を挙げている。特に千差万別の形態を持つている側篩骨洞Recessus lateralisの粘膜除去は，上顎洞経由或は鼻内よりの侵襲のみを以てしては不完全と認めざるを得ない幾多の症例を発見した。私共は現在に於ては前頭洞の広く発達した症例は勿論，前頭洞の殆んど欠損し篩骨洞Recessus frontalis内に埋没した例に於ても眉毛内切開による侵襲を試みてRecessus lateralisの部の病的粘膜除去の徹底を期している。

　Hishiyama and Uyeno advocate the use of hexamethonium chloride, an agent that causes reduction of blood pressure, for prevention of excessive hemorrhage or maintaining hemorrhage at the minimum during operative procedure on paranasal sinuses. This agent is administered in dosage of or ampules intravenously immediately prior to the operation and thereafter, the patient's head should be elevated by inclination of the operating table. The degree in the attainment of lowered-blood-pressure may be controled by the degree of the angle in which the table may be inclined: at the same time the undesirable shock of excessive lowering of blood pressure may thus, be avoided.

　The authors state that they have yet to meet untoward side-effects from the use of this agent as well as post-operative hemorrhages: the use of the agent is quite safe.
