顔面整形特に鼻整形のルート

　近来鼻整形或いは顔面整形術は，頓に盛んになり，吾々耳鼻科医も之に無関心であり得ない状態である。而して所謂鼻整形のルートとしては，主として鼻前庭よりプロテーゼを挿入する方法が専ら行われている。又外傷特に打撲による内眥部及び上顎骨前頭突起部の陥凹には，犬歯窩より頬部を剥離し，プロテーゼを挿入する方法が用いられているのが普通である。然るに強力な外力に依り招来された鼻根部附近の複雑な醜形，即ち所謂鞍鼻式斜鼻を呈し，然も内眥部前頭突起部に陥凹を来しているものに対しては，上記の二方法を各々二次的に行うのを通例としているが，両者に同時に適合する如き（写真の様な）プロテーゼが，一次的に挿入出来れば如何に便利であるかと考えていた。所が最近偶々2,3の症例に遭遇し，之に新しい試みを用いて極めて容易に成功，患者の満足を得たので，鼻整形に対する一つの新しい方法であると考え，茲に報告し，御批判を仰ぎ度いと思う。

Hishiyama and associates advocate, in facial deformities particularly the nasal where cor-rections are highly necessary, the use of ciliary approach whereby the initial incision is placed longitudinally in the eyebrow. Through this incision the skin of the area involved is elevated from the underlying structures to afford insertion of proper material of prosthesis. This procedure in contrast to that using canine fossa is devoid of leaving an ugly incision scar as the after effects of the operation, and, moreover, the involved area is more readily accessible.
