扁摘術について考えている事ども

　吾が耳鼻咽喉科領域に於て，もつともひんぱんに行われている手術は口蓋扁桃摘出術であろう。扁摘術については，現在ほぼその全貌を語りつくされたかの如くであるが，しかも尚意見の一致を見ていない点もあるようである。即ち扁桃被膜の問題については，その見解は諸家の問で尚一致せず，又その他後出血の問題，年齢的適応，或いは術後の疼痛の問題等も尚未解決として残されているようである。之等の諸点について吾々が日頃考えている事をのべ，諸賢の御批判を仰ぎたいと思う。

Hishiyama and Uyeno give the outline of their method of performing tonsillectomy with dis-cussions on the optimum age of the patient at which this operation may be performed. Their study is mainly centered on the character of tonsil capsule which is composed of parts clo-sely adherent to underlying tissues and parts that are somewhat loosely distributed. Enuc-leation of the tonsillar tissues is performed in a most ideal way when separation be attained within this capsular tissue. The hemorrhage at the time of operation should be controled by pressure application on the cavity wall ra-ther than, attempting to ligate various bleeding points. For alleviation of postoperative pain the use of irigapirin intramuscularly is effe-ctive.
