中耳形成手術の基本型

　慢性化膿性中耳炎及びその残遺症の治療は第1には炎症の消失と再燃の防止であり，第2には聴力の恢復を計るというにある。而して炎症の消失は手術に際して洗滌，吸引を行いながら操作を行い，拡大鏡下に注意深く操作することによつて，その目的の達せられることは既に報告^1）した通りである。

　第2の目的である聴力の恢復については既に報告^1）した通りであるが，慢性中耳炎の病変は極端に云えば症例毎に異つている。そのために聴力を恢復せしめるたるには夫々異つた手術操作を必要とするのである。然しながら音伝導機構の再製法については，その主なるものを如何にするかと云うことであつて，これには病変と関連して大きく数型に分類され得るのである。これらの型を基本型として症例毎に少しく操作を変えて応用すれば，すべての症例に於いてその目的を達し得るのである。かかる見地からして，ここに中耳形成手術の基本型について記して見ることとする。

Goto states that operative procedures which aim at reconstruction of conduction mecha-nism in treatment of chronic otitis media may be divided into 6 different basic types. The mode of the procedure depending upon the object directed towards elimination of in-flammatory process on one hand and recovery of hearing loss on the other naturally differ in accordance to circumstance met with in di-fferent cases but, profound attention is given to conditions that may prevail among such stru-ctures as tympanic membrane, middle ear ossi-cies, oval window and the auditory tube. Postoperative progress and the degree with whichhearing recovery was obtained are listed for each type of operation. Theoretical aspects of these procedures are discussed.
