中耳形成術に於ける鐙骨の臨床的意義

　慢性中耳化膿症の治療としての中耳形成術は，炎症の消退と聴力の恢復を目的としている。そこで本手術に於ては病変除去と伝音機構再製という2つの操作が必要である。

　然らば耳小骨連鎖が遮断されているものに於ては，如何にして伝音機構を再生さすべきかという問題がある。これについては二方面からの研究がある。即ち伝音機構の理論から出発してそれに適合する手術方法を行うことがその1つであり，他の1つは臨床的に種々の手術方法を行つて聴力恢復を観察することである。その理由は慢性中耳炎に於ける伝音障碍は復雑な因子によつて起つており，正常耳の伝音機構に於ける理論のみからは判定し得られないことが少くないし，他方臨床的観察からしてかかる伝音障碍に於けるその再生を知り，これから逆に伝音機構破礎時の伝音並に伝音理論を新しく考察し得るからである。

Goto states that, from the study of state of stapes found in chronic otitis media, he is inclined to disagree with contentions of Kley who maintains that recovery of postoperative hearing pariticularly of that of high-tones is influenced by the condition presenting prese-nce or absence of os stapedius. The. author is of opinion that regardless of that condition of stapes, if surgical interveniton is instiuted ea-rly in the couse of otitis medea the recovery of hearig becomes usually the case.
