緒言

　最後部副鼻洞（蝶形洞又蝶形洞性篩骨蜂窠）を鼻内的に手術する際其の視野の最深部の外上方に屡骨壁の限局性の隆起を認める事がある．之の骨壁隆起は古くはZuckerkandl，Onodiによつて視束（視神經）の通過する爲めに生じた副鼻洞骨壁の隆起とされ，主に解剖學的に2，3檢討さているが臨床的には只鼻性視神経經炎との關係のみが假設的に觸れられているだけで手術的には殆んど顧みられていない．

　私はこの骨壁隆起に就て解剖的にも臨床的にも檢索した所後者に於て特に手術との關係が甚だ密接であり尚鼻性視神經とも重要な關係にあるのを知り得たので之等に就ての概要を述べたいと思う．

　TAKAHASHI Proposes a new terminology Eminentia optica to be applied to the portion of the bony wall which may protrude into the extreme posterior paranassal sinus as tuberosity that covers the canalis fasciculi optici. The importance of recognition of the structure in operative procedure is stressed in that it may be essential for the diagnosis and treatment of some visual disorders. In order to expose i freely for removal intranasal approach is the method of choice. Emminentia optica may be found to be constiuting a close relation to the seat of optic neuri is of nasal origin.
