 The surgical decompression of the optic nerve through the ethmoidal sinuses was administered to 19 patients with complaints of post traumatic visual disturbances. The criteria for effectiveness of decompression surgery were: 1) visual acuity 0~sensus luminis → motus manus or more, 2) motus manus → numerous digitorum or more, 3) numerous digitorum → visual acuity 0.01 or more, 4) visual acuity 0.01~0.09 → more than 0.02 improvement, and 5) visual acuity 0.1 or more → more than 0.2 improvement. The surgery was effective for 15 patients (78.9%). There was no ineffective cases in 11 patients who had surgery within 2 weeks after the trauma, whichsuggested the importance of early surgical treatment.

 No correlation was found between the degree of rising of the optic canal, or the presence or absence of the fracture of the canal and the result of the surgery.

 As to the three patients out of four for whom the surgery was ineffective, more than a month and a half had elapsed before operation, andatrophy of the papilla nervi optici was observed. It was assumed that organic changes of the edema of the optic nerve resulted in degenerative changes.

 The surgical decompression of the optic nerve through the ethmoidal sinuses is not technically difficult. It was suggested that otorhinologists familiar with the anatomy of the paranasal sinuses would perform this operation more positively.
