I.緒言
BarnettおよびPeerless4)らが中心となって行われたEC-IC bypass surgeryに関するco-operative studyの結果は北米の脳外科医のみならず,われわれ日本の脳外科医にも少なからずのショックを与えたようである1,2).著者は,このco-operative studyのメンバーの一員にさせて頂いていたので,原則的に,一般的に,この結果はacceptしている.
しかしながら著者らは,いまだに,数は限られているが,このbypass surgeryを脳梗塞例に行っており,今後とも続ける方針でいる.なぜなら現在まで得られた自験例の中に,completed strokeでありながらbypasssurgeryの翌朝に誰が見ても症状の改善をみた症例があるからである.それは,ただ脳外科医のみならず,看護婦,患者の家族,患者自身(そして,ときには神経内科医すら)も認めざるを得ないほど,有意な臨床症状の改善を示す症例が厳然とあるという臨床家の実感によるものである.
The role of EC-IC bypass surgery has become a very controversial issue recently. EC-IC anastomosis was performed in 132 patients of stroke in our institute.
Thirty six of them which could be followed up com-pletely had a completed stroke and ten of these showed a significant clinical improvement by the next day of surgery. We studied the angiograms, CT scans and dynamic CT studies of all 36 patients to determine the common factors responsible for a significant clinical im-provement in these ten patients.
