I．緒言

　著者らの施設にCTが導入されて以来，約10年が経過した．その間，経験した高血圧性脳出血は610例である．この10年間，個々のCTを検討し，あるものは手術を行い，そして，幸いにして「脳卒中友の会」という著者らの施設で治療した患者よりなる親睦団体を通じて，長期間，比較的患者に密着してのlong-term followupを行う機会を得た．ここらで，CT所見を中心にした高血圧性脳出血の臨床病態，手術適応，そして最近の著者らの実験的検討を，このlong-term follow upを基にして一度まとめてみたいと思う．その第1報として，本稿では被殻出血について，その血腫進展様式とlong—term outcomeとの相関を中心にして，被殻出血の臨床病態について著者らの検討結果を紹介し，諸賢の御批判を頂きたいと思う．

A retrospective analysis of our series of 355 cases of putaminal hemorrhage revealed some interestingaspects of its pathogenesis. All the cases were studied in detail with special attention to clinical presentation, diagnostic findings and the long-term clinical outcome. Role of surgery in management of putaminal hemor-rhage is still controversial. Inability to form proper useful classification has been one of the reasons responsible for this controversy. A new classification is formulated which divides putaminal hemorrhage into four types.
