Ⅰ．はじめに

　脂肪塞栓症候群は外傷，ことに骨折の重大な合併症であり，主として整形外科領域においてよく知られ報告も多い．一方，本症候群はその60-70％^3,9）と高率に脳症状を伴い，しかも外傷後一定のfree interval（無症状期）を経て発症してくるので，多発外傷例においては頭蓋内血腫との鑑別が重要であり，その意味において脳神経外科医も本症候群を認識しておく必要がある．

　脂肪塞栓症候群は欧米において約1世紀前より多くの報告があり，決して稀な疾患ではないが，本邦においての報告は少なく，50例前後の症例報告を見るのみである．また，神経学の分野よりの本症候群の報告は欧米においても少なく^{5,17,18,33）}，本邦では金谷^10），中山^11），山城ら^27）の6例と荒木^1）によるもののみであり，神経学的側面からの検討はいまだ充分になされてはいない．著者らは剖検例を含む3症例を経験したので，本症候群の臨床診断，特に脳症状について検討を行い報告する．

In spite of many case reports of fat embolism syndrome (FES) in Europe and in the USA, only 50 cases were reported in Japan. Most of the reports were made by orthopedists or by general surgeons. Considering the scarce description on FES by neurologist or neurosurgeon in Japan and also in other countries, the authors report three cases of FES and analyze neurological signs and symptoms in these cases.

Case 1: 17 years old man was crushed in his both legs by motor cycle accident. He lost his consciousness right after the accident and he was transferred to the Kitasato University Hospital.
