Ⅰ．はじめに

　頭蓋内類上皮腫は全脳腫瘍の約1％^{6，8，12，16，33）}であり，類皮腫はさらに稀で0.1-0.4％^12，33）と報告されている．中枢神経系の全類上皮腫および類皮腫の約1／4は脊椎管内にみられ，この類皮腫には先天性皮膚洞に伴うものが高頻度に認められている^24）．

　我々の教室では最近の10年間に頭蓋内類上皮腫4例（全脳腫瘍手術例の0.57％）と類皮腫1例（0.14％），および洋椎管内類上皮腫1例が手術されている（Table 1）．これら6例のうち，嚢種内結石のみられた後頭蓋窩類上皮腫例，先天性皮膚洞に伴った脊推管内類上皮腫例および本邦ではいまだ報告されていない第4脳室内類皮腫例など興味ある症例を中心に報告し，また類上皮腫と類皮腫との病理発生の違いについても考察する．

Six cases of epidermoid and dermoid cyst in the central nervous system were experienced in our department from 1966 to 1975. This report presented unusual findings in three cases out of the six. This first case of epidermoid cyst in the posterior fossa, was characterized by the presence of a stone in the cyst, the second case of intraspinal epidural epidermoid cyst by the coexistence of congenital dermal sinus at the lumbo-sacral region, and the third case of the dermoid cyst by its location in the fourth ventricle.
