Ⅰ．まえがき

　本稿では紙数の関係で，鎮痛薬の中での標準薬ともいうべきmorphineの鎮痛作用を中心に記述することにしたい。

　morphineの鎮痛作用発現機序の研究は，1）電気生理学的手法による作用部位の研究，2）生化学的手法によるレセプター研究，morphineの分布，morphineと化学伝達物質との関係などに関する研究，3）morphineの脳内注射による仮性疼痛反射抑制を指標とする神経薬理学的研究などに大別され，それぞれの方法による知見が互いに補いあいながら前進しつつある。したがって1種類の方法に拠った知見でmorphineの作用機序を記述することは不可能といってよい状況にある。

　Recent advances on the site of action of morphine were reviewed. Effect of morphine on the brain stem, particularly the nucleus reticularis giganto cellularis (NRGC) were investigated. NRGC appears to be more sensitive area to morphine than periaqueductal gray matter. It is postulated that morphine enhances the descending inhibitory influence from NRGC to the spinal dorsal horn (lamina V cell). Besides this action, morphine has a direct suppressive action on the spinal dorsal horn (lamina V cell).
