臨床からみた"いたみ"の問題点

Some problems on pain from the clinical aspects

　いたみは臨床医学の中でも最も関心の深い症状の一つで，その歴史は古く，これまでに多くの研究が積み重ねられている。いたみについての考え方は研究の進歩とともにいろいろ変わってはきているが，これら多くのものは，体性感覚としてのいたみについてのものである。ヒトが訴えるいたみには体性感覚の考え方では理解しがたい別のいたみがあり，これらを混同して取り扱っているところに，いたみの最も大きな問題点がある。ここでは，このような，臨床からみたいたみの問題点について，いくつかの点をとりあげ，述べることにした。

Pain is a great topic in medicine and has been studied from the points of various aspects by many workers. Big results have been obtained, but many subjects to be discussed and studied still remain.

In this paper, some problems on pain from the clinical aspects will be picked up and discussed.

Sensory pain and emotional pain Sensory pain is elicited by conduction of pain stimulus in the sensory pathway which consists of sensory receptor, peripheral sensory nerve, spinal cord, brain stem, thalamus and cerebral cortex.
