脳卒中治療の戦略的展開―医療経済学的視点からの考察

Strategies for fighting against “stroke epidemic” in this century: Importance of economic evaluation.

超急性期に真に有効な治療手段の登場は，brain attack時代とも呼称される脳卒中治療の新時代をもたらした。本邦の脳卒中診療も，画像診断技術の進歩による病態把握の充実や，無症候性脳血管障害の診断増加など大きく変貌しており，この変化に如何に対処するかが課題となっている。また治療面でも，抗血小板療法，脳保護薬などの薬物療法や血管内膜剥離術，血管内治療法などの外科的治療法の進歩が見られ，その適応決定や治療効果の的確な評価法の確立が求められている。

これらの進歩は，まさに脳卒中治療戦術の多様性をもたらしている。しかしながら，これらの新しい診断・治療技術は適切な対象に適切な時期に無駄なく用いられてこそ，その真価を発揮する。その意味で，限られた財源を各治療戦術の充実に効率よく振り分けるために，医療経済学的視点を踏まえた治療戦略の構築とその展開が重要である。本稿ではその基本となる事項を中心に解説を試みた。

Stroke is not only an enormous public health problem, but also an enormous public financial problem in most of the countries including Japan, where an impressive reduction of stroke mortality has been established in a couple of decades of the last century but a dramatic increase of stroke patients is expected in concomitant with continuous increase of older-age population in the begining of this century.
