総合外来における胸痛関連症状患者の診療に関する記述的研究

The Descriptive Study of the Ambulant Care for Chest Pain Related Patients

　総合外来を初診する胸痛関連症状の患者に対して，どのような診療がなされているかの現状を記述し，適切な診療が行われるための基礎資料とすることが本研究の目的である．1990年7月12日～8月9日に当院総合外来を初診した1,000人の全外来患者を対象とし，このうち胸痛関連症状で受診したものを循環器専門医が抽出した．そのカルテから年齢，性，症状の始まり，解釈モデルの一部，初診医，初診時診断などを転記した．104人（10.4％）が胸痛関連症状で受診した．年齢，性に差はなかった．胸痛，胸苦しさ，胸部圧迫感などの6症状のうち2症状がある患者を重症感のあるものと定義すると，この28人の群では心電図施行率が89.4％とそうでない患者に比べて有意に高かった．診療の希望を記載した患者は63人で，このうち医学的説明を求めたものが42人と最も多かった．何も希望を記載していない37人の心電図施行率が62.6％であるのに対して，この42人では40.4％と有意に低かった．総合外来では症状から診断するアプローチがよく使用されていると考えられた．リクエストから患者にアプローチすることも含めた胸痛関連症状の患者への適切な診療が求められた．

Objective : To describe the present practice for chest pain related patients and to collect data for the consideration of further appropriate clinical tests.

Patient population : 1,000 patients, visiting on Jichi Medical School Omiya Medical Center from July 12th to Aug. 9th 1990, were studied. One cardiologists selected those who complained of chest pain related symptoms. Several dermographic and medical data were gathered from their medical records.

Results : 104 patients (10.4%) visited with 33 chest pain related symptoms. There were no significant differences in age and sex.
