　症例は53歳，女性。排尿時下腹部不快感，下腹部腫瘤の主訴で膀胱鏡，CT,MRI検査を施行し，膀胱頂部に粘膜の浮腫状変化を伴う粘膜下腫瘍を認め，尿膜管腫瘍の疑いで生検を施行した。悪性所見は認めなかったが，画像診断上悪性腫瘍を否定できず試験切開し，膀胱壁術中迅速診断を施行したがやはり悪性所見は認めず，膀胱黄色肉芽腫性炎症と診断された。比較的稀な症例で，鑑別は困難であった。

A 58-year-old woman presented with complaints of lower abdominal discomfort and bladder irritation. Physical examination revealed a fist size mass in the hypogastric area, adherent to deep structure, without tenderness. Gynecological study was normal. Abdominal computed tomography and magnetic resonance imaging showed a 8.0 ×7.0cm mass connected with umbilicalis, and suspected to adhere to rectal muscle. Tumor marker and urinary cytology were negative and percutaneus needlebiopsy showed chronic inflammation.
