膀胱癌骨転移症例の臨床的検討

Clinical Studies on Bone Metastases from Bladder Cancer

　浸潤性膀胱癌患者の癌死が確認された症例58例を対象とし，骨転移症例に関して検討した。癌死症例のうち剖検あるいは生検などで骨転移が確認されたのは14例（24％）であった。膀胱全摘除術後の再発が10例であり，温存療法後に4例発見された。14例中3例は治療後何らかの術後補助化学療法施行後5年以上経過してからの再発であるが，残りの11例は骨転移出現後2か月以内に癌死した。また，治療後5年以上経過して経口抗癌剤中止後に骨転移が出現した症例が2例存在した。これらより膀胱癌の骨転移に対する化学療法により予後が延長する症例の存在することが示唆された。

Clinical data were analysed for 58 patients with bone metastases who died by advanced bladder cancer. Fourteen out of 58 patients were detected bone metastases by biopsies or autopsies. Ten patients were identified after radical cystectomy and four patients after bladder preservation therapy. Three patient recurred more than 5 years after receiving adjuvant chemotherapy. Rest of all other patients died within 2 months. Late bony metastases appeared in two patients after oral anticancer drugs were stopped.
