陰茎壊疽性膿皮症の1例

A Case of Pyoderma Gangrenosum of Penis

　38歳，男性。陰茎亀頭部に米粒大の丘疹を認め，次第に自潰し亀頭部破壊および前部尿道瘻を形成したため当科を受診した。細菌検査など諸検査は全て陰性であり，病変部の病理組織診断では非特異的炎症であった。以上より壊疽性膿皮症と診断し，ステロイドによる治療を開始したところ，潰瘍は次第に軽快したため4か月後に終了した。以後再発はみられず，亀頭部の欠損はみられるが尿道からの排尿，性交および射精も可能である。

A 38-year-old man presented with penile papule and ulceration. Swabs examinations for microbacteria were negative and skin biopsy revealed non specific inflamma-tion. Pyoderma gangrenosum was the final diagnosis, since purulent ulceration caused by neoplasia, malignancy, Fournier's gangrene were ruled out. Treatment was dif-ficult despite the use of high doses of prednisolone and minocycline. As a result, glans penis was partly destroyed leaving urethral stricture. Surgical treatment was per-formed, and the patient restored normal urination and sexual intercourse without recurrence of the pyoderma gangreno-sum.
