　68歳，男性。発熱を主訴に近医受診し急性腎盂腎炎の診断で入院加療を受けるが軽快せず，精査加療を目的に当科受診。左側腹部に腫瘤を触知した。画像診断にて腎周囲膿瘍を疑ったが腎腫瘍を否定できず左腎摘除術を施行。病理組織学的に黄色肉芽腫性腎盂腎炎の診断であった。本症は3型に分類されており（膿腎症型，腎膿瘍型，腎周囲型），今回の症例は腎周囲型であった。腎周囲型の本症は稀で，本邦第4例目に相当すると思われる。

A 68-year-old man was admitted to Nihon University, Surugadai Hospital with a complaint of fever. Physical examination revealed a fist sized mass in his left flank, and radiologically, left perine-phric abscess was suspected, but the possibility of renal tumor was not neglected. After left nephrectomy was performed, histological diagnosis revealed xanthogranulomatous pyelonephritis. Xanthogranulomatous pyelonephritis is classified in 3 types (pyonephrotic type, renal abscess type, perinephric type) and perinephric type is quite rare. This case is the 4th case of perinephric type reported in Japan.
