　53歳，女性。肉眼的血尿，頻尿，排尿痛を訴えて受診した。膀胱鏡では非乳頭状の腫瘍を認めた。尿細胞診では小細胞型と扁平上皮型の2種類の異なる悪性細胞が検出された。右外腸骨周囲リンパ節では小細胞癌の転移がみられたが，膀胱腫瘍の経尿道的生検では扁平上皮癌のみしか認めなかった。総合的に考えて，膀胱の扁平上皮癌を伴う小細胞癌であると診断した。患者は初診後4か月で癌死した。

A 53-year-old-female consulted our hospital complaining gross hematuria, pollakisuria and painfull urination. Cys-toscopy revealed non-papillary bladder tumor. Urine cytol-ogy revealed two different type of malignant cell, namely, small cell and squamous cell type. Resected external iliac lymph nodes were diagnosed as metastasis of small cell carcinoma but bladder tumor, transurethrally obtained was squamous cell carcinoma. Our final diagnosis was small celll carcinoma with squamous cell carcinoma of the urinary bladder. The patient died 4 months after the initial consulta-tion to our hospital.
