結石を伴った女性尿道憩室腫瘍

Carcinoma Arising from Female Urethral Diverticula with Stones:A Case Report

　73歳，女性。尿閉を主訴に入院した。KUBおよび排泄性尿路造影では恥骨上に2個の石灰化像を認め，尿道憩室内の結石と考えた。引き続き行った尿道膀胱鏡検査，CT,MRIおよび膀胱頸部にみられた腫瘍の生検の結果，結石を有する尿道憩室より発生した腫瘍であると診断した。抗癌剤の動注療法ののち，膀胱尿道全摘除術を施行した。摘除標本では，尿道憩室は2個の憩室口を有しているものと思われた。術後1年を経過した現在，再発を認めていない。

A 73-year-old female was admitted to our hospital com-plaining urinary retention. KUB and excrete urogram showed two calcifications on the pubic bone, which were thought to be stones in the urethral diverticulum. Ureth-rocystoscopy, CT scan, MRI and transurethral biopsy of the bladder neck tumor were successively perfomed, and carcinoma arising from urethral diverticulum with stones was disclosed. After the arterial infusion of anti-cancerous drugs, radical cystourethrectomy was perfomed. Resected specimen showed that there were two diverticular orifices. One year after the operation, the patient is alive without disease.
