過酸化水素水注入が奏効したナットクラッカー現象による左腎出血の1例

A Case of Left Renal Bleeding Due to Nutcracker Phenomenon Treated by Hydrogen Peroxide Instillation

　32歳，男性。肉眼的血尿を認めたため当科を受診した。膀胱鏡にて左尿管口より血性尿線を認め，諸検査の結果，ナットクラッカー現象による左腎出血と診断した。止血剤などにより経過観察したが，肉眼的血尿は続いた。約30か月の時点で過酸化水素水の腎盂内注入を行ったところ，肉眼的血尿は消失した。

A 32-year-old man was admitted to our hospital with a complaint of gross hematuria. Cystoscopy documented hematuria from the left ureteral orifice. We diagnosed left renal bleeding due to nutcracker phenomenon. After ob-servative treatment for 30 months the patient underwent retrograde hydrogen peroxide instillation to the renal pelvis. Since then, gross hematuria was not found.
