緒言

　睾丸付属小体は，睾丸または副睾丸に付着もしくはその付近に存在する胎生期の遺残物であり，その付着部位により①睾丸垂appendix testis，②副睾丸垂appendix epididymis，③睾丸傍体para-didymis，④迷管vas aberransに分けられている^2）。

　睾丸付属小体捻転症の本邦における最初の報告は，井上が1933年に睾丸垂捻転症を睾丸小泡体転捩症の1例として発表しており，以来大熊ら5）は本邦文献上23例を集計している。その内訳は，睾丸垂捻転症13例，副睾丸垂捻転症10例であり，睾丸傍体と迷管の捻転症例の報告は現在まで見当らない。今回われわれは，4歳男児にみられた睾丸傍体捻転症の1例を経験したので報告する。自験例は本邦文献上第1例目であり，欧米例を含めると4例目である。

A 4-year-old boy was admitted with swelling and pain in the right side of the scrotum 4 days in duration. General physical examination was negative except for the genitalia. The right side of the scrotum was reddend, edematous and tender to palpation. Puncture of the right side of the scrotum revealed bloody fluid. Preoperative diagnosis was the tumor in the scrotum. Upon opening of the tunica vaginalis, the bloody fluid was evacuated. The testis and epididymis ware normal. A black small mass of little finger tip size with white-colored pedicle 2 mm in length was attached to the spermatic cord 1 cm above the head of the epididymis.
