副睾丸捻転症

TORSION OF EPIDIDYMIS

　9歳，男児。友人に陰嚢部を蹴られ，6日後下腹部痛と陰嚢部痛を訴え，8日後症状悪化し当科受診。右陰嚢部は著しい発赤腫脹あり，小鶏卵大の腫瘤として触れた。確定診断は得られず試験切開した。副睾丸の頭体部が睾丸より遊離し，反時計方向に3回転半捻転しており，副睾丸捻転症であることが判明した。病理組織学的に副睾丸組織の形態は残しているが，ほとんどすべてが出血壊死に陥っていた。

A 9-year-old boy noticed pains in the lower abdomen and scrotal region which was kicked six days before. Right scrotum was remarkably swelled with reddish color and was palpated as a tumor in the size of a small egg. Operation was made on un emergent basis. The head and body regions of the epidi-dymis was separated from the testicle and were rotated 3. 5 times counterclockwise, and the diagnosis of torsion of epididymis was made pathohistologically, although epididymal tissue was remained, almost all fell into hemorrhagic necrosis.
