DOUBLE CONTRAST RETROGRADE URETHROCYSTOGRAPHY (D-RUC)

緒言

　X-rayを利用して男性の尿道の病変を検出する主な方法はRetrograde Urethrography（RU）とEvacuation Cystourethrography（ECU）とがある^1）。RUは主として前部尿道，ECUは主として後部尿道の検査であるともいえるが，後者は特に外来の患者に対して不便な点があるので実際に一般に最もよく使用されているのは前者のRU方法である。しかし，RU方法にしてもECU方法にしても，その膀胱内に注入した造影剤によつて膀胱頸部とこれに隣接するProstatic Urethraの一部は造影剤がかさなつてよく描きだされないことが多い。FlocksのAir CystographyとRetro-grade UrethrographyとのComination方法^2）はこの膀胱頸部をよく描きだす方法であるが，これもしばしば膀胱頸部はその注入した造影剤とかさなつて詳細な読影ができない場合がある。このために著者はこれとよく似ている方法を考案した。これは特に前立腺肥大症と前立腺癌に対する鑑別診断に非常によい方法であることを経験し，この方法（Double Contrast Retrograde Urethrocysto-graphy：略D-RUC）とこれによる症例のX-ray　filmsを従来のRU法によるX-ray filmsと比較しながら紹介し，読者の御批判を得たいと思う。

A radiological method of detecting lesion at the posterior urethra, especially the bladder neck and the prostatic urethra immediately adjacent to it, is reported. This method, which is named as Double Contrast Retrograde Urethrocystography (D-RUC) is similar to that devised by Flocks. With the use of this double contrast method, the clinical prostatic carcinoma, which often involves the bladder neck and the prostatic urethra adjacent to it, and which is most likely to be overlooked by the con-ventional retrograde urethrography, can be detected.
