緒言
近年腎動脈撮影の急速な進歩によつて腎出血を来す一つの原因である腎動静脈瘻の診断も容易になつて来た。Varela1)が1928年に最初の腎動静脈瘻の症例を報告して以来,多数の症例報告がつづいたが,特に近年腎動脈撮影の普及によつてこの方面の症例報告も急速にふえて来た2〜11)。腎動静脈瘻は先天的と腎外傷,生検,悪性腫瘍の浸潤,動脈瘤の破裂,腎摘後などによる後天的なものがある。外傷,生検などによる後天的なものに対しては,その動静脈瘻と関連のある動脈の結紮や腎部分切除などの外科的な療法が行なわれている12,13)。しかし,先天的なものはそのFeedingarteriesの本管は単に1本だけではなく,ほとんど全部のSegmental arteriesから来た方が多いので,外科的な療法としては腎摘除を要する報告の方が多い。われわれは最近3例の先天的な腎動静脈瘻による血尿の症例を経験した。このうちの1例は約1週間のベット上安静で血尿が消失したが他の2例は手術を要した。ここにその2例についての治療方法を報告したい。
Two patients with congenital renal arteriovenous cirsoid fistulas presenting only continuous gross hematuria are described. Both of them were treated by ligating the segmental artery which was considered to be the chief arterial feeder of the fistulas. Hematuria stopped immediately after the operation. Good function was retained in the operated kidney. No adverse sequelae to the operation have been observed after more than a year of follow up study. No hematuria recurred after the operation though the postoperative follow up arteriography showed that a considerable amount of cirsoid fistulas still remained unchanged.
