緒言

　近年，前立腺癌，特にStage　ⅡとⅢの前立腺癌に対し前立腺全摘術を行う回数が増えてきたが，この手術は視野が狭くまた血管が多いため，依然として出血量の多い難しい手術のひとつとされている。

　われわれは1977年本誌に恥骨後式前立腺精嚢全摘術とその止血方法を報告したが^1），最近この方法に改良を加えた結果，術中出血量を減少させ手術時間も短縮させることができたので，ここにこの改良した方法と手術成績を報告したい。

In our previous report we dealt with way to reduce the intraoperative hemorrhages by reversing the conventional procedure of retropubic total prostatovesiculectomy. Instead of amputating the prostato-urethral junction first, the bladder neck was directly amputated first and from where seminal vesicles and posterior prostatic capsule were dissected toward the posterior aspect of the prostatourethral junction. This could really offer a wide and dry operative field much better than that by conventional procedure of this operation.
