恥骨後前立腺剔除術の新しい止血法と手術成績

A NEW HEMOSTATIC METHOD IN RETROPUBIC PROSTATECTOMY

緒言

　前立腺肥大症（以下BPH）に対する手術療法としては，もつとも多くおこなわれているのはTUR，恥骨上式，恥骨後式，会陰式という順序である^1）。しかし，術中と術後の出血の多いことがこの手術の問題点であつて，内科の泌尿器科医によつて幾多の工夫改良がなされている。われわれもこの点について努力し，よい結果を得たので報告する。

　またこれに関して最近欧米と日本との報告のうち，その手術成績についてふれた論文は次のものがある（第1表）。

The problem of massive hemorrhages during and after prostatectomy still places a determent to most of the urologists in the surgical treatment of prostatic hypertrophy. Various hemostatic methods for reducing blood loss, during and after operation, have been devised.

A new method (fig.4) of hemostatic suture-ligation on the prostatic bed in retropubic prostatectomy is reported. Retropubic approach is preferred for it causes little postoperative bladder irritation than the suprapubic one which is often panicking the patient with troublesome postoperative fistulas and tenes-mus.
